We welcome families who value discipline, excellence, and opportunity. Our admissions process allows us to understand each student’s potential and ensure they are ready to succeed both in the classroom and in athletics.
• Birth Certificate (original must be presented in the office)
• Two recent 2x2 photos
• Social Security card (original must be presented in the office)
• Official transcript from previous school
• Letter of recommendation from previous school
• Medical form
• PVAC-3 (Original Green Form – Immunization Certificate). Students without this form will not be admitted. Required by the Department of Health.
• Form SO-001 (required for grades Kinder, 2nd, 4th, 6th, 8th, and 10th). Oral examination certificate required by the Department of Health.
• If the student has asthma, an Asthma Action Plan completed by their doctor or specialist is required. Required by the Department of Health.
Important Note: All required documents must be submitted and included in the student’s file on or before the first day of classes. Any student missing required documentation will not be allowed to begin classes until all documents are submitted, without exception.
